1. Happy House Fall 2025 Registration Form:

• When you [click] the form link below you will be taken to the new Happy House Nursery and Pre-K Fall 2025 Registration form. This is a printable (pdf) file that you may print out, complete and submit as directed for your child to be considered for enrollment this coming Fall 2025 semester. 2. Forms Required before your child attends school.

• Before your child can attend school in the Fall of 2025, you will be required to Open, Complete and Return the NYS Day Care, Medical Statement, Release Authorization, and Acknowledgement  forms below. Please make sure the Medical form is signed on the back page by your child’s health care provider before returning.   If your child stays for the full day program, you will also need to fill out the Napping Agreement.

• We will be offering a Before and After School program at the Elmira Happy House site only.   Forms will be available in August.
